You may mail it to Worcester Urological Assoc, 25 Oak Ave, Worcester, MA 01605, or if you have access to a fax machine, you may fax it to 508-756-9404. We will contact you when your records are ready for pick up. There is no charge for this. We have truly enjoyed caring for you through the years and wish you good health for the future.
